After Birth: The Postpartum Truth Nobody Tells You
Share
"Jab baccha paida hota hai — kamra mubarak baad se bhar jaata hai. Aur mama? Mama akele baith kar aansu poochhti hai aur kehti hai — main theek hoon. Lekin tum theek nahi ho. Aur yeh bilkul theek hai."
When a baby is born in Pakistan, the room fills with mithai, relatives, and celebration. Everyone crowds around the newborn — taking photos, arguing over who the baby looks like.
And the mother? She sits quietly. Aching. Exhausted. Bleeding. Confused by emotions she cannot name. And when someone asks how she is feeling — she says what every Pakistani woman says: Main theek hoon. I am fine.
She is not fine. And this blog is for her.
The Fourth Trimester — The Season Nobody Talks About
The 'fourth trimester' is the 12 weeks after birth. Your baby is adjusting to the world outside the womb. And you — your body, your mind, your identity — are going through the most profound transition of your life.
In Pakistan, this period is called the Chilla — 40 days of rest, warmth, and recovery. It is one of our most beautiful traditions. But Chilla ends at 40 days. The fourth trimester is 12 weeks. That leaves 44 days where you are expected to be 'back to normal' — while your body and hormones are still rebuilding entirely.
What Your Body Is Going Through
- Postpartum bleeding (lochia) can last 4–6 weeks — heavier than any period you have had
- Your uterus is contracting back to size — painful, especially during breastfeeding
- If you had a C-section: recovering from major abdominal surgery on zero sleep
- Perineal pain if you had a vaginal birth — sitting, walking, even standing hurts
- Hormones drop dramatically in the first 3–5 days — this is why the baby blues hit
- Hair loss starts at 3–6 months postpartum — normal but deeply distressing
- Night sweats as your body releases retained pregnancy fluids
💛 Mama Tip: The most important thing postpartum is REST. Not cooking. Not entertaining. Not proving you are managing. REST is medicine. Treat it like medicine.

Breastfeeding — The Beautiful, Brutal Reality
"Nobody told me it would hurt this much. Nobody told me I would cry every time he latched. Nobody told me how desperately I would try."
Breastfeeding is the most natural thing in the world — and one of the hardest things many mothers will ever do. In Pakistan, the pressure to breastfeed exclusively is enormous. And when it does not come easily — and for many women it does not — the guilt is crushing.
The Most Common Breastfeeding Problems Pakistani Mamas Face
- Milk not coming in for the first 1–3 days — colostrum is there even if you cannot feel it
- Cracked and bleeding nipples — the most common reason women stop in week one
- Engorgement — breasts so full and hard they ache constantly
- Low supply anxiety — the most common breastfeeding fear in Pakistan
- Mastitis — breast infection causing fever, pain, and flu symptoms
- Pumping at work — no private space, no break time, no support
Every single one of these has a solution. You do not have to give up. But you do not have to suffer in silence either.

Postpartum Mental Health — The Conversation We Must Have
1 in 3 Pakistani new mothers experiences postpartum depression or anxiety. That is 33%. That is not a small number. That is your friend, your sister, your neighbour — and possibly you.
Postpartum depression in Pakistan is deeply misunderstood — confused with weakness or ingratitude. In reality, it is a hormonal condition that requires care, not judgment.
Signs You May Need Support
- Crying for no clear reason, multiple times a day
- Feeling disconnected from your baby — like you are watching from outside
- Intrusive thoughts about harm — to yourself or the baby
- Feeling like you have made a terrible mistake
- Inability to sleep even when the baby is sleeping
- Rage that feels disproportionate and uncontrollable
"Postpartum depression does not mean you are a bad mother. It means you are a mother who is unwell and deserves care. Maangna help ka haq hai aap ka. You have the right to ask for help."
If you recognise yourself in any of these — please tell your doctor, your partner, or someone you trust. At FirstBreath, we are not just selling products. We are building a community where Pakistani mamas can say 'I am struggling' — and be met with support, not silence.
"Yeh waqt guzar jaye ga. Ye raatein guzar jaengi. Yeh dard guzar jaye ga. Aur ek din — woh tum par muskurae ga — aur tum bhool jao gi sab. Bas woh muskurahat yaad rahe gi. Hamesha."
